“Your biological age is 47.” It sounds precise, but the science behind biological age is becoming less straightforward.

Chronological age counts years. Biological ageing describes changes in cells, tissues and physiological systems—and those changes do not necessarily happen at the same pace.

Biological Age: What Does the Number Actually Measure?

Biological-age tests use different biomarkers to estimate aspects of ageing. Epigenetic clocks, for example, analyse DNA methylation patterns. Different clocks measure different features, so they can produce different results for the same person.

Why Different Organs Age at Different Rates

A new Nature Aging study makes this distinction clearer. Researchers analysed 25,306 post-mortem tissue samples from 40 tissue types in 970 donors aged 21–70. Using a method called PathStAR, they found three broad patterns of organ-specific aging: some tissues changed more rapidly in the 30s, others accelerated later, while nine of 14 analysed tissues showed two periods of faster structural change.

Vascular tissues showed earlier structural changes, while the uterus and vagina accelerated later, around menopause. Digestive and male reproductive tissues showed biphasic patterns. The researchers also found evidence of coordinated deterioration between related tissues.

The Limits of Epigenetic Clocks

This does not make existing clocks obsolete. A separate Nature Medicine analysis examined 51 human intervention studies, comparing 16 epigenetic clocks and 94 additional DNA-methylation biomarkers. Some biomarkers responded more consistently to interventions, but the researchers stressed that responsiveness is not the same as a validated clinical outcome.

The emerging picture is therefore less about finding one definitive biological age and more about building a biological profile—one that may eventually describe how different systems are changing, rather than reducing ageing to a single number.

The tissue study was cross-sectional and based on post-mortem samples; it cannot determine how an individual person's organs will age.
